If an employee injury is reported to you,

1. Assess if 911 services are needed. If so, call 911 first

2. Call the Manager on Duty, if no answer, call CSOC 650-874-6692

3. Notify UROC 650-874-7200

4. If employee is transporting to hospital, complete as much of the OIS report as possible.

5. If employee is not transporting to hospital, complete the entire report and provide the employee a copy of the necessary forms on the OIS page.

6. A copy of this report must be sent to our Wellness Office upstairs in Administrative Office Area.
